Home Care Coordination

Senior Care Consultants, Inc. – Providing Private Geriatric Care Managers.

We provide situational assessments, coordination, and management for loved ones.

Our independent, professional, and third-party options give a second voice to the adult child trying to manage their parents’ lives, as well as their own. We work with you to develop a customized care plan that meets the needs of your parent or loved one.

If you have an aging parent, but are separated geographically, we can help you manage the home care needs of your parents, right here in Pittsburgh. We also assist in-town children who are busy with their careers and families.

Our geriatric care managers can consult with you by phone to help evaluate your unique situation, listen to your goals and needs, and develop a plan of care for your parent or loved one.

Senior Care Managers also:

  • Monitor the level of care given
  • Coordinate household chores, meals, maintenance, bill paying and banking
  • Assist with medication management
  • Accompany the client to doctors’ appointments
  • Assist in obtaining and coordinating home care, nursing home care, and care in retirement or extended care facilities
  • Offer guidance to out-of-town family members and act as a liaison between the family, older adult, physician, attorney and others
  • Advocate for clients’ and families’ needs
  • Reassess and reevaluate your long-term health plan

In addition, The Care Registry offers several nurse aide services from qualified, caring workers. Our list of senior home care services
